Narrative:
Upon entering the cockpit I went through the normal checklists for the start of a flight. Maintenance had the aircraft log book so I was unable to review the log at this time. Everything was normal except for 1 placard on one of the IRS's. About 15 mins prior to departure maintenance returned the log book. I was busy loading the flight plan into the FMC, so the captain reviewed the log. He concurred that one of the IRS's was inoperative and checked the MEL and determined that we were legal to go since it was the #2 IRS that was placarded. The flight left on time. Upon reaching cruise altitude of 35000', I was flying this leg, the captain did a position check. While doing this check he discovered that the IRS that was placarded and therefore off was not the #2 IRS as was written up in the log book, but was the #3 IRS which we had already entered oceanic control airspace. Fortunately the #2 IRS was working properly. The flight continued west/O any trouble. The IRS's in the widebody transport are numbered left to right 1,3,2. Maintenance mistakenly put the placard on the #3 IRS. They have no #'south on them and were probably thought to be numbered. Should have been more careful in checking which IRS was inoperative. Also, they should have numbers on the IRS so this type error does not happen again.

Title: MECHANIC PLACARDS WRONG NAVIGATION SET INOPERATIVE RESULTING IN VIOLATION OF MINIMUM EQUIPMENT LIST REQUIREMENT.
Narrative: UPON ENTERING THE COCKPIT I WENT THROUGH THE NORMAL CHKLISTS FOR THE START OF A FLT. MAINT HAD THE ACFT LOG BOOK SO I WAS UNABLE TO REVIEW THE LOG AT THIS TIME. EVERYTHING WAS NORMAL EXCEPT FOR 1 PLACARD ON ONE OF THE IRS'S. ABOUT 15 MINS PRIOR TO DEP MAINT RETURNED THE LOG BOOK. I WAS BUSY LOADING THE FLT PLAN INTO THE FMC, SO THE CAPT REVIEWED THE LOG. ALSO, THEY SHOULD HAVE NUMBERS ON THE IRS SO THIS TYPE ERROR DOES NOT HAPPEN AGAIN.
