Narrative:

I had driven from plymouth, ma, to westerly, ri, that evening, where my aircraft is based in westerly, ri, in order to check out some repair work on the replacement of an alternator and fuel pressure sending unit completed by company of wst. I took off runway 25, reached pattern altitude and made my turn for my left downwind landing sequence. I announced to wst unicom, 123.0, 'turning left downwind to runway 25 at westerly,' and repeated this communication for left base and final. I monitored all engine and aircraft parameters, ran through the gump sequence, turned the fuel booster pump on and set up for the T/D on 25. The next moment I realized the gear was not down. I did not hear the gear up warning horn--which was later checked out at company to be intermittently faulty. Contributing factor: fatigue. Prevention measures: 1) install a visible 'eye level' landing sequence checklist on instrument panel; 2) perform an in-flight gear down warning horn check more frequently; 3) when going through the checklist for landing, call out the sequence out loud to myself as performed checking indication.

Title: GEAR UP LNDG. NO WARNING HORN SIGNAL.

Narrative: I HAD DRIVEN FROM PLYMOUTH, MA, TO WESTERLY, RI, THAT EVENING, WHERE MY ACFT IS BASED IN WESTERLY, RI, IN ORDER TO CHK OUT SOME REPAIR WORK ON THE REPLACEMENT OF AN ALTERNATOR AND FUEL PRESSURE SENDING UNIT COMPLETED BY COMPANY OF WST. I MONITORED ALL ENG AND ACFT PARAMETERS, RAN THROUGH THE GUMP SEQUENCE, TURNED THE FUEL BOOSTER PUMP ON AND SET UP FOR THE T/D ON 25. THE NEXT MOMENT I REALIZED THE GEAR WAS NOT DOWN. I DID NOT HEAR THE GEAR UP WARNING HORN--WHICH WAS LATER CHKED OUT AT COMPANY TO BE INTERMITTENTLY FAULTY. CONTRIBUTING FACTOR: FATIGUE. PREVENTION MEASURES: 1) INSTALL A VISIBLE 'EYE LEVEL' LNDG SEQUENCE CHKLIST ON INSTRUMENT PANEL; 2) PERFORM AN IN-FLT GEAR DOWN WARNING HORN CHK MORE FREQUENTLY; 3) WHEN GOING THROUGH THE CHKLIST FOR LNDG, CALL OUT THE SEQUENCE OUT LOUD TO MYSELF AS PERFORMED CHKING INDICATION.
