Narrative:
Captain completed customs and company formalities with ramp agent outside of aircraft. First officer obtained ATC clearance while captain was attending to company matters. First officer read back and wrote down clearance correctly first officer failed to enter transponder code. Flight taxied out and was instructed to contact departure when airborne. After takeoff departure was contacted and advised aircraft was 'idented.' flight was instructed to 'maintain runway heading, climb to FL250 upon passing 13000'.' captain heard another aircraft instructed to make, 'immediate turn to 010 degrees by approach controller. Captain increased visibility scan and observed an medium large transport pass down left side within 1/2 mi. Subsequent discussing with approach revealed our flight was a primary target only as computer couldn't tag flight due to different transponder code. First officer could offer no explanation as to why he read back code but did not set it in. Approach controller admitted forgetting about our flight as he dismissed us as a VFR target. Company commenced action to monitor first officer's action as regards personal performance.

Title: LTT TRANSPORT TAKING OFF CANADIAN ARPT FAILED TO ACTIVATE PROPER CODE IN MODE C TRANSPODER. CTLR HAD PRIMARY TARGET ONLY, CONSIDERED THE TARGET A VFR--CONFLICT ONE HALF MILE WITH MLG.
Narrative: CAPT COMPLETED CUSTOMS AND COMPANY FORMALITIES WITH RAMP AGENT OUTSIDE OF ACFT. F/O OBTAINED ATC CLRNC WHILE CAPT WAS ATTENDING TO COMPANY MATTERS.
