Narrative:
After checking in with chicago approach; the supervisor asked us what arrival we were on and we stated fissk two. Then asked us about our transition and if we were familiar with the fissk NOTAM. We were not so we re-reviewed (briefly since we were on approach) the weather pack and discovered the fdc NOTAM on page 5. We were flying the charted procedure of halie intersection then 292 track to hills. The NOTAM buried on page 5 is 270 track to cgt then 335 heading. Cgt is not even depicted on the chart! I didn't really carry on the conversation with ATC since we were concerned with flying the approach and all of my responses were very brief. It was not the time or place to be discussing our error. We proceeded to land uneventfully. After landing; I called dispatch to let them know about our issue; so they could warn other fissk arrivals. Bottom line; I missed the fdc NOTAM. We had a MEL issue. The weather had been bad the previous two days and today was an absolutely beautiful day causing me to lower my guard. I learned my lesson. If this NOTAM has been around for a while the chart needs to be changed or eliminated! Developing procedures and then modifying them for long periods of time is asking for safety issues! We should not be filed on this arrival until it is corrected. This should have been on the cover page of the weather pack and even on the ATIS.

Title: B737 flight crew reports being informed they are off track on the FISSK 2 RNAV to MDW due to not being aware of a NOTAM modification to the procedure.
