Narrative:
Aircraft came in the previous night with #2 tank fuel quantity gauge inoperative. This requires load verification via dripsticks and certification on company form xx by 2 people. The tanks were 'dripped' but the certification did not appear on the xx. We were in a slight rush and I was somewhat tired after a busy week and missed the lack of certification on the paperwork.

Title: FLT CREW FAILED TO VERIFY FUEL LOAD FORM CERTIFICATION PRIOR TO DEP.
Narrative: ACFT CAME IN THE PREVIOUS NIGHT WITH #2 TANK FUEL QUANTITY GAUGE INOPERATIVE. THIS REQUIRES LOAD VERIFICATION VIA DRIPSTICKS AND CERTIFICATION ON COMPANY FORM XX BY 2 PEOPLE. THE TANKS WERE 'DRIPPED' BUT THE CERTIFICATION DID NOT APPEAR ON THE XX. WE WERE IN A SLIGHT RUSH AND I WAS SOMEWHAT TIRED AFTER A BUSY WEEK AND MISSED THE LACK OF CERTIFICATION ON THE PAPERWORK.
