Narrative:
I observed an untagged discrete beacon code approximately 3 mi and 12 O'clock to air carrier X's position. I issued a traffic alert and air carrier X responded that he had traffic in sight. Subsequent to the near midair collision, it was discovered that the unknown aircraft was on an IFR flight plan. It was small transport Y that was misplaced by another controller.

Narrative: I OBSERVED AN UNTAGGED DISCRETE BEACON CODE APPROX 3 MI AND 12 O'CLOCK TO ACR X'S POSITION. I ISSUED A TFC ALERT AND ACR X RESPONDED THAT HE HAD TFC IN SIGHT. SUBSEQUENT TO THE NMAC, IT WAS DISCOVERED THAT THE UNKNOWN ACFT WAS ON AN IFR FLT PLAN. IT WAS SMT Y THAT WAS MISPLACED BY ANOTHER CTLR.
