Narrative:
Aircraft X, an small aircraft, was on CCC 057 right at 5000' without mode C. Atx Y, an light transport, was on CCC 057 right at 4000 gaining on slower small aircraft. Aircraft X was told to descend and maintain 4000 and report reaching by controller a. After being told to descend, controller a relieved controller B and position briefing was incomplete. Atx Y was turned, but not enough. Controller B thought aircraft X was still at 5000', due to wrong altitude in data block. Entry by controller a showing 5000. FAA is investigating the incident.

Title: CTLR A DESCENDED ACFT X TO SAME ALT AS ATX Y AND NMAC OCCURRED. SYSTEM ERROR.
Narrative: ACFT X, AN SMA, WAS ON CCC 057 R AT 5000' WITHOUT MODE C. ATX Y, AN LTT, WAS ON CCC 057 R AT 4000 GAINING ON SLOWER SMA. ACFT X WAS TOLD TO DSND AND MAINTAIN 4000 AND REPORT REACHING BY CTLR A. AFTER BEING TOLD TO DSND, CTLR A RELIEVED CTLR B AND POSITION BRIEFING WAS INCOMPLETE. ATX Y WAS TURNED, BUT NOT ENOUGH. CTLR B THOUGHT ACFT X WAS STILL AT 5000', DUE TO WRONG ALT IN DATA BLOCK. ENTRY BY CTLR A SHOWING 5000. FAA IS INVESTIGATING THE INCIDENT.
