Narrative:
I was giving a VFR only commercial pilot instruction toward his instrument rating. (He has some 1600 hours total time and is a glider CFI.) because of nice VMC conditions, the student was to fly approachs under the hood while I acted as safety pilot/instrument. Elmira approach and departure control was handling our approachs to ith. We had been cleared for the ith ILS runway 32 approach and were approximately 1.5 mi inside the OM (varna) when I realized we had not been given the usual frequency change and radar service terminated statement. I tuned in ith's tower frequency and heard the end of a transmission from ith: 'I'm not talking to that airplane, sir.' my scan then went from cautious and careful to near panic. I then saw an aircraft (small aircraft Y) coming right at us off runway 14. I took command of the airplane, had my student remove the hood, and made a climbing right turn. After a few words with an un-flustered ith tower, the training mission continued. My student and I are convinced we did not get a 'radar service terminated, contact ith tower' transmission on that first approach. Because we waited for that frequency change, we flew into an air traffic area and into the departure path of another aircraft believing we were following ATC directions. I believe the controller's workload was so great, he forgot us. Next time, I'll query ATC when past the LOM on an approach west/O a frequency change.

Title: APCH CTL FORGETS TO SWITCH LIGHT VFR ACFT ON INSTRUMENT TRAINING FLT OVER TO THE LCL CTLR AT THE OUTER MARKER--INSTRUCTOR AND STUDENT FORGET TO ASK. AIRBORNE CONFLICT RESULTS.
Narrative: I WAS GIVING A VFR ONLY COMMERCIAL PLT INSTRUCTION TOWARD HIS INSTRUMENT RATING. (HE HAS SOME 1600 HRS TOTAL TIME AND IS A GLIDER CFI.) BECAUSE OF NICE VMC CONDITIONS, THE STUDENT WAS TO FLY APCHS UNDER THE HOOD WHILE I ACTED AS SAFETY PLT/INSTR. ELMIRA APCH AND DEP CTL WAS HANDLING OUR APCHS TO ITH. WE HAD BEEN CLRED FOR THE ITH ILS RWY 32 APCH AND WERE APPROX 1.5 MI INSIDE THE OM (VARNA) WHEN I REALIZED WE HAD NOT BEEN GIVEN THE USUAL FREQ CHANGE AND RADAR SVC TERMINATED STATEMENT. NEXT TIME, I'LL QUERY ATC WHEN PAST THE LOM ON AN APCH W/O A FREQ CHANGE.
