Narrative:

An IFR aircraft departed spi and entered stl approach control airspace west/O prior coordination. The problem was discovered by the pilot and controller simultaneously. The aircraft was then picked up by stl approach control. The spi controller was decertified on the control position. No evasive action was required. OJT training was being conducted, radar beacon targets were intermittent and maintenance was researching and working on the problem. Aircraft transponder was intermittent and no primary target continued throughout the flight; it also was intermittent. The aircraft proceeded RNAV to a point 8 northeast of the troy VORTAC prior to initiating any communication with ATC.

Title: ACFT ENTERED ADJACENT FAC AIRSPACE WITHOUT A HANDOFF.

Narrative: AN IFR ACFT DEPARTED SPI AND ENTERED STL APCH CTL AIRSPACE W/O PRIOR COORD. THE PROB WAS DISCOVERED BY THE PLT AND CTLR SIMULTANEOUSLY. THE ACFT WAS THEN PICKED UP BY STL APCH CTL. THE SPI CTLR WAS DECERTIFIED ON THE CTL POS. NO EVASIVE ACTION WAS REQUIRED. OJT TRNING WAS BEING CONDUCTED, RADAR BEACON TARGETS WERE INTERMITTENT AND MAINT WAS RESEARCHING AND WORKING ON THE PROB. ACFT XPONDER WAS INTERMITTENT AND NO PRIMARY TARGET CONTINUED THROUGHOUT THE FLT; IT ALSO WAS INTERMITTENT. THE ACFT PROCEEDED RNAV TO A POINT 8 NE OF THE TROY VORTAC PRIOR TO INITIATING ANY COM WITH ATC.
