Narrative:
While climbing thru FL240; [we] received pack 1 vlv clsd message on EICAS. Performed emerg/abnorm procedure and message went away. Previous write-ups exist; dating back to log page XXXXX06; item # 1; in july 2014. While reviewing aircraft logbook for previous occurrences; found a previous maintenance station write-up indicating ram air duct blown out; needs new duct (log page XXXXX16; item # 3). There is no further documentation in aircraft logbook indicating this was corrected. MEL 21-29 (pack 1) was cleared approximately six days later on log page XXXXX18; item # 3. [But log page XXXXX16; item # 3 still open]. Logbook entry incomplete; additional documented discrepancies not corrected.
