Narrative:
Departed the site to continue the flight back to our base. The problem arose after returning to base when I realized that in my haste to return that I had failed to make a proper logbook entry and signoff showing what I had done, as well as the aircraft being airworthy prior to departing for base. After arriving at the hospital a proper entry was made.

Title: ACFT DEPARTED WITHOUT MAINTENANCE ACTION SIGN OFF IN ACFT LOGBOOK.
Narrative: DEPARTED THE SITE TO CONTINUE THE FLT BACK TO OUR BASE. THE PROBLEM AROSE AFTER RETURNING TO BASE WHEN I REALIZED THAT IN MY HASTE TO RETURN THAT I HAD FAILED TO MAKE A PROPER LOGBOOK ENTRY AND SIGNOFF SHOWING WHAT I HAD DONE, AS WELL AS THE ACFT BEING AIRWORTHY PRIOR TO DEPARTING FOR BASE. AFTER ARRIVING AT THE HOSPITAL A PROPER ENTRY WAS MADE.
