Narrative:
Cleared C4 approach to rio from scr. Captain did not have approach plate out. On long final approach reclred us to R09; again no plate. Captain did not know where he was in relation to runway 09. I gave him visibility clues to runway but he aligned with 09 at 4000' AGL. He started to descend then turned left 360 degrees. During turn tower reclred us to cax VOR at 4500' straight at an incoming X air carrier widebody transport. No more than 500' vertical clearance nose to nose.

Narrative: CLRED C4 APCH TO RIO FROM SCR. CAPT DID NOT HAVE APCH PLATE OUT. ON LONG FINAL APCH RECLRED US TO R09; AGAIN NO PLATE. CAPT DID NOT KNOW WHERE HE WAS IN RELATION TO RWY 09. I GAVE HIM VIS CLUES TO RWY BUT HE ALIGNED WITH 09 AT 4000' AGL. HE STARTED TO DSND THEN TURNED LEFT 360 DEGS. DURING TURN TWR RECLRED US TO CAX VOR AT 4500' STRAIGHT AT AN INCOMING X ACR WDB.
