Narrative:
Air carrier X and a general aviation flight were head-on at 11,000. Air carrier X departed cvg and climbed to 11,000 nebnd. The general aviation aircraft was swbnd at 11,000 when the data block timed out, which I believe was a major factor in the separation error. I was the manual controller at the cvg lo sector. When the error happened I was on the landline coordinating releases for departures of cvg, day, and cmh. The sector was very busy and we were using a tracker. Supplemental information from acn 123552: I put pvd J ball around air carrier X. Controller knew radar separation was diminishing rapidly. I told him to turn the aircraft. Air carrier X was turned east to 090 degree. Small aircraft Y was in second 360 degree turn of which he made a left turn into air carrier X.

Title: ACR CAME IN CLOSE PROX TO ANOTHER IFR ACFT.
Narrative: ACR X AND A GENERAL AVIATION FLT WERE HEAD-ON AT 11,000. ACR X DEPARTED CVG AND CLIMBED TO 11,000 NEBND.
