Narrative:
Upon departure began to climb off end of runway. PF unsure of assigned heading and attempted to check with PNF. PNF said heading which did not seem to match departure. Some questioning went on about this and both pilots looked at departure. At this time climb to altitude was amended from 23,000 to 10,000. Still distracted by conflicting heading and departure information, we tried to verify heading with departure and were unable due to faulty microphone of PNF. PF had to make radio call and flew through altitude. Departure reassigned altitude and heading. Company paperwork was received with cavet-1 departure and was programmed in FMS by PNF. PF received clearance and was given san francisco 3 departure which was not put in the FMS. Departure clearance should have been discussed more thoroughly on the takeoff briefing.

Title: ALT BUST ON DEP. FMC NOT REPROGRAMMED FOR CLRNC RECEIVED AND CREW NOT SURE OF HEADING REQUIRED ON DEP.
Narrative: UPON DEP BEGAN TO CLIMB OFF END OF RWY. PF UNSURE OF ASSIGNED HDG AND ATTEMPTED TO CHECK WITH PNF. PNF SAID HDG WHICH DID NOT SEEM TO MATCH DEP. SOME QUESTIONING WENT ON ABOUT THIS AND BOTH PLTS LOOKED AT DEP. AT THIS TIME CLIMB TO ALT WAS AMENDED FROM 23,000 TO 10,000. STILL DISTRACTED BY CONFLICTING HDG AND DEP INFO, WE TRIED TO VERIFY HDG WITH DEP AND WERE UNABLE DUE TO FAULTY MICROPHONE OF PNF. PF HAD TO MAKE RADIO CALL AND FLEW THROUGH ALT. DEP REASSIGNED ALT AND HDG. COMPANY PAPERWORK WAS RECEIVED WITH CAVET-1 DEP AND WAS PROGRAMMED IN FMS BY PNF. PF RECEIVED CLRNC AND WAS GIVEN SAN FRANCISCO 3 DEP WHICH WAS NOT PUT IN THE FMS. DEP CLRNC SHOULD HAVE BEEN DISCUSSED MORE THOROUGHLY ON THE TKOF BRIEFING.
