Narrative:
I was flying with a new first officer for the first time. ZOA had cleared us for a runway 28 profile descent into sfo with a 250 KT speed restriction at the 44 DME fix. This profile descent has a minimum crossing restriction at 60 DME off oak VOR. I had tuned in sfo VOR 115.8 to aid him on his approach with the DME off of sfo. He assumed I had tuned in oak VOR 116.8 for the profile radial and west/O checking his profile chart. He also tuned in 115.8. At the 60 DME fix off of oak and sfo, the RMI needles and the HSI are close enough together so that with a new first officer, I did not suspect anything, nor did I check his navigation frequency. When we neared the 44 DME fix and were slowing to 250 KTS, ZOA asked what our altitude had been at the 60 DME fix, as there was a 16000' restriction. I remember that we had crossed at what I thought was 8000', because it was the first crossing restriction and I thought he was a little high. Center then asked what VOR we were on, and I realized my mistake. I do not know what our altitude was at the 60 DME fix. My mistake in this situation was not telling the first officer what frequency I was on and why I was on it, and also not monitoring his actions more closely on a profile with several restrictions.
Original NASA ASRS Text
Title: FLT CREW OF LGT FAILED TO MAKE CROSSING RESTRICTION ON PROFILE DESCENT INTO SFO.
Narrative: I WAS FLYING WITH A NEW F/O FOR THE FIRST TIME. ZOA HAD CLRED US FOR A RWY 28 PROFILE DSCNT INTO SFO WITH A 250 KT SPD RESTRICTION AT THE 44 DME FIX. THIS PROFILE DSCNT HAS A MINIMUM XING RESTRICTION AT 60 DME OFF OAK VOR. I HAD TUNED IN SFO VOR 115.8 TO AID HIM ON HIS APCH WITH THE DME OFF OF SFO. HE ASSUMED I HAD TUNED IN OAK VOR 116.8 FOR THE PROFILE RADIAL AND W/O CHKING HIS PROFILE CHART. HE ALSO TUNED IN 115.8. AT THE 60 DME FIX OFF OF OAK AND SFO, THE RMI NEEDLES AND THE HSI ARE CLOSE ENOUGH TOGETHER SO THAT WITH A NEW F/O, I DID NOT SUSPECT ANYTHING, NOR DID I CHK HIS NAV FREQ. WHEN WE NEARED THE 44 DME FIX AND WERE SLOWING TO 250 KTS, ZOA ASKED WHAT OUR ALT HAD BEEN AT THE 60 DME FIX, AS THERE WAS A 16000' RESTRICTION. I REMEMBER THAT WE HAD CROSSED AT WHAT I THOUGHT WAS 8000', BECAUSE IT WAS THE FIRST XING RESTRICTION AND I THOUGHT HE WAS A LITTLE HIGH. CENTER THEN ASKED WHAT VOR WE WERE ON, AND I REALIZED MY MISTAKE. I DO NOT KNOW WHAT OUR ALT WAS AT THE 60 DME FIX. MY MISTAKE IN THIS SITUATION WAS NOT TELLING THE F/O WHAT FREQ I WAS ON AND WHY I WAS ON IT, AND ALSO NOT MONITORING HIS ACTIONS MORE CLOSELY ON A PROFILE WITH SEVERAL RESTRICTIONS.
