Narrative:

Mlt X, a helicopter, called 10 mi south balance at 1300'. I radar idented mlt X and instructed him to remain west of runway 15R for departure traffic, and to report 1 1/2 mi south. Mlt X was told to plan mid-field crossing. Mlt X reported 1/2 mi south, was issued departure traffic, an medium large transport on runway 15R. I thought pilot had traffic, but tapes revealed pilot said, 'looking.' air carrier Y was issued traffic ahead to right helicopter mid-field crossing at 1200' and cleared for takeoff. Air carrier Y acknowledged. Helicopter was still northbound and appeared to be passing behind air carrier Y when he turned eastbound and passed overhead ar Y lifting off. Air carrier Y stated traffic was too close and later filed a near miss.

MLT X RPTED 1/2 MI S, WAS ISSUED DEP TFC, AN MLG ON RWY 15R. I THOUGHT PLT HAD TFC, BUT TAPES REVEALED PLT SAID, 'LOOKING.' ACR Y WAS ISSUED TFC AHEAD TO RIGHT HELI MID-FIELD XING AT 1200' AND CLRED FOR TKOF. ACR Y ACKNOWLEDGED. HELI WAS STILL NBOUND AND APPEARED TO BE PASSING BEHIND ACR Y WHEN HE TURNED EBND AND PASSED OVERHEAD AR Y LIFTING OFF. ACR Y STATED TFC WAS TOO CLOSE AND LATER FILED A NEAR MISS.
