Narrative:
In 3/90 T lax ARTCC over the las VOR, 2 fgt X mistook a clearance for another aircraft and descended head-on into air carrier Y that just departed las. Prior to this incident the receivers on one side failed and the backups were not usable either. This resulted in basically no communication with aircraft. Supervisor was notified prior to incident. This situation could've been avoided if equipment was updated. Frequency failure is becoming a daily thing. Supplemental information from acn 139115: 2 fgt X's descended on a head-on collision course with air carrier Y. The underlying reason for this error was lack of communications. 5 mins prior to this error the radios went bad (main and backup). It was very difficult to relay xmissions. Apparently the fgt X pilot took a clearance that was not for him.

Title: FGT X DESCENDED FROM ASSIGNED ALT AND HAD LESS THAN STANDARD SEPARATION FROM ACR Y. PLT DEVIATION.
Narrative: IN 3/90 T LAX ARTCC OVER THE LAS VOR, 2 FGT X MISTOOK A CLRNC FOR ANOTHER ACFT AND DSNDED HEAD-ON INTO ACR Y THAT JUST DEPARTED LAS. PRIOR TO THIS INCIDENT THE RECEIVERS ON ONE SIDE FAILED AND THE BACKUPS WERE NOT USABLE EITHER. THIS RESULTED IN BASICALLY NO COM WITH ACFT. SUPVR WAS NOTIFIED PRIOR TO INCIDENT. THIS SITUATION COULD'VE BEEN AVOIDED IF EQUIP WAS UPDATED. FREQ FAILURE IS BECOMING A DAILY THING. SUPPLEMENTAL INFO FROM ACN 139115: 2 FGT X'S DSNDED ON A HEAD-ON COLLISION COURSE WITH ACR Y.
