Narrative:
While we, mdt bizjet X, were taxiing for takeoff at bct, an small aircraft Y called 1/2 mi final, and 1 other aircraft, ant small transport Z, was noticed on downwind, not yet abeam the approach end of runway 5, where we were by now. Small aircraft Y crossed the threshold for landing as we turned left, up to the hold line for runway 5. I told the first officer to announce that we were taking the active for departure, which he did on 122.8. While lining up on runway 6, we realized that small aircraft Y was doing touch and go's, and was airborne again, so we held our position waiting for him to turn crosswind. Instead, he continued to climb straight out. We then heard small transport Z announce that he was on a 1 mi final, and asked, 'aircraft on the runway at boca, are you going to roll?' the copilot responded, on 122.8, 'as soon as the departing small aircraft Y turns crosswind.' (it must be noted here that I heard all xmissions made by the copilot on 122.8, but they sounded weaker than normal.) at this time, small aircraft Y initiated his left turn to crosswind, so I advanced the power levers toward takeoff power. While I was doing so, small transport Z announced that he was going around. Within 3 seconds or so, we saw him pass to our right at less than 100' AGL, then bank left and proceed to fly down the centerline of the runway while climbing slowly. By this time I had reached takeoff power, but was holding the brakes, now because of small transport Z. As he reached what must have been at least 2/3 of the way down the runway (3500' or so), I released the brakes to begin the takeoff roll. I was anticipating his turn for crosswind, but he didn't make it until we had rolled 1600' or so. As a result, we passed behind and below him. It was when the copilot switched frequencys and initiated a call to palm beach approach, and FBO told us that we were still on their frequency, that we realized that he had had the wrong transmit button depressed the entire time. The problem was that I was hearing all the copilot's xmissions as bleed through on the 122.8 frequency on #1 radio, but he was transmitting on #2. This has been determined to be due to an audio panel problem. Had I not heard his xmissions at all, or had we been talking to a tower or ground controller, we would have been aware of the improper audio panel switch selection immediately. Apparently the other aircraft in the pattern never heard any of our transmission. Needless to say, the audio panel is being repaired. Other contributing factors, however, were: small aircraft Y not initiating his turn to crosswind at a reasonable time after takeoff--particularly since the airport remarks section in the airport/facility directory listing for boca raton, fl instructed aircraft departing runway 05 to turn left within 1/2 mi; small transport Z delaying his go around so long (that he passed within 100' of us), when there was another aft on the runway who was clearly (from his perspective) having a communications problem; and my failure to delay my takeoff by a few seconds--enough time to determine exactly what small transport Z was going to do.

Title: CORP MDT DEPARTING FROM UNCONTROLLED ARPT, BUT MISTAKENLY NOT ON CORRECT CTAF, HAS TO HOLD IN POSITION LONGER THAN EXPECTED, FORCING SMT ARRIVAL INTO A CRITICAL GND CONFLICT GO AROUND.
Narrative: WHILE WE, MDT BIZJET X, WERE TAXIING FOR TKOF AT BCT, AN SMA Y CALLED 1/2 MI FINAL, AND 1 OTHER ACFT, ANT SMT Z, WAS NOTICED ON DOWNWIND, NOT YET ABEAM THE APCH END OF RWY 5, WHERE WE WERE BY NOW. SMA Y CROSSED THE THRESHOLD FOR LNDG AS WE TURNED LEFT, UP TO THE HOLD LINE FOR RWY 5. I TOLD THE F/O TO ANNOUNCE THAT WE WERE TAKING THE ACTIVE FOR DEP, WHICH HE DID ON 122.8. WHILE LINING UP ON RWY 6, WE REALIZED THAT SMA Y WAS DOING TOUCH AND GO'S, AND WAS AIRBORNE AGAIN, SO WE HELD OUR POS WAITING FOR HIM TO TURN XWIND. INSTEAD, HE CONTINUED TO CLB STRAIGHT OUT. WE THEN HEARD SMT Z ANNOUNCE THAT HE WAS ON A 1 MI FINAL, AND ASKED, 'ACFT ON THE RWY AT BOCA, ARE YOU GOING TO ROLL?' THE COPLT RESPONDED, ON 122.8, 'AS SOON AS THE DEPARTING SMA Y TURNS XWIND.' (IT MUST BE NOTED HERE THAT I HEARD ALL XMISSIONS MADE BY THE COPLT ON 122.8, BUT THEY SOUNDED WEAKER THAN NORMAL.) AT THIS TIME, SMA Y INITIATED HIS LEFT TURN TO XWIND, SO I ADVANCED THE PWR LEVERS TOWARD TKOF PWR. WHILE I WAS DOING SO, SMT Z ANNOUNCED THAT HE WAS GOING AROUND.
