Narrative:
On climb out lost pressurization. Master warning for cabin altitude illuminated. Performed emergency procedure and notified ATC and began a descent. Descended to 10;000 ft MSL. Received clearance back to [the departure airport]. Upon inspection it was found that cargo floor retaining ring had failed allowing pallet jack to roll back and hit rear bulkhead. The bulkhead appeared to be damaged and be the cause of pressurization loss.
Original NASA ASRS Text
