Narrative:
While on takeoff roll, air noise started in the vicinity of first officer sliding window and became louder as speed increased. Takeoff was rejected at 120 KTS. Crew discovered sliding window was locked by handle; but not completely closed in rear by crank. This is an item that should have been discovered on the checklist. The plane was delivered to crew from maintenance hangar where someone had forced lock handle in place west/O completely closing window. First officer did not check line-up stripe, only lock handle. Perhaps all crew members should monitor one another in accomplishment of checklist items. At least this captain will initiate this procedure. Better training of mechanics would help also. Supplemental information from acn 151898. After brake cooling and a maintenance check of brakes trip was resumed and completed uneventfully.
Original NASA ASRS Text
Title: ACR WDB TKOF ABORTED DUE OPEN SLIDE WINDOW.
Narrative: WHILE ON TKOF ROLL, AIR NOISE STARTED IN THE VICINITY OF F/O SLIDING WINDOW AND BECAME LOUDER AS SPD INCREASED. TKOF WAS REJECTED AT 120 KTS. CREW DISCOVERED SLIDING WINDOW WAS LOCKED BY HANDLE; BUT NOT COMPLETELY CLOSED IN REAR BY CRANK. THIS IS AN ITEM THAT SHOULD HAVE BEEN DISCOVERED ON THE CHKLIST. THE PLANE WAS DELIVERED TO CREW FROM MAINT HANGAR WHERE SOMEONE HAD FORCED LOCK HANDLE IN PLACE W/O COMPLETELY CLOSING WINDOW. F/O DID NOT CHK LINE-UP STRIPE, ONLY LOCK HANDLE. PERHAPS ALL CREW MEMBERS SHOULD MONITOR ONE ANOTHER IN ACCOMPLISHMENT OF CHKLIST ITEMS. AT LEAST THIS CAPT WILL INITIATE THIS PROC. BETTER TRNING OF MECHS WOULD HELP ALSO. SUPPLEMENTAL INFO FROM ACN 151898. AFTER BRAKE COOLING AND A MAINT CHK OF BRAKES TRIP WAS RESUMED AND COMPLETED UNEVENTFULLY.
