Narrative:
I instructed air carrier X to taxi into position and hold on runway 32 behind air carrier Y (who was landing). Air carrier Z was cleared to land on the same runway. From that time on I draw a blank as to what was happening. I remember looking at air carrier X on the runway and wondering to myself why the aircraft was not rolling. When I asked the pilot, he stated that I had not cleared him for takeoff. Using a wrong call sign (air carrier Y). I told air carrier Z to stand by for a go around after I had cleared air carrier X for takeoff. Air carrier X departed and was airborne and air carrier Z elected to go around. It turned out that there was a minimum 6000' sep situation but it was an accident (not planned). I was not distracted at the time because I was looking at the air carrier X wondering why he was so slow. I cannot remember air carrier Y landing or turning off the runway. I remember air carrier Y crossing the runway threshold. Overall, I don't think this was a good operation, nor did it look good out the window. I have no suggestions for preventing this in the future other than trying to be more alert. In my mind, I felt I had cleared air carrier X for takeoff. I have not listened to the tape and at this point am unclear as to whether I did or did not.

Title: ACR Z FLT CREW PERCEPTION LESS THAN STANDARD SEPARATION FROM ACR X.
Narrative: I INSTRUCTED ACR X TO TAXI INTO POS AND HOLD ON RWY 32 BEHIND ACR Y (WHO WAS LNDG). ACR Z WAS CLRED TO LAND ON THE SAME RWY. FROM THAT TIME ON I DRAW A BLANK AS TO WHAT WAS HAPPENING.
