Narrative:
After returning from a 135 trip of 3 1/2 hours, I turned in the clipboard for the aircraft. Before leaving on the previous trip I checked all the paperwork really good to see if everything was correct. I checked the paperwork extremely closely because the clipboard was a new one because the old one had been lost. As I was saying, after the trip, I turned in the clipboard and started talking to the dispatcher and other employees. While I was talking to them I looked on the status board and found that the status board had not been updated to match the clipboard. Also, the new dispatcher said the 100 hour had not been done yet. After checking with the maintenance director the 100 hour had not fully been done, but started. Also the form showing that the 100 hour was completed was showing that it had been finished. I guess someone must have filled the new one out thinking that the 100 hour had been done. How the problem arose: from the loss of the clipboard with the documents in it. How it was discovered: it was discovered through conflicting times on the status board and the clipboard sheet. Contributing factors: lack of aircraft and quick maintenance turnarounds. Corrective actions: filling our NASA form, alerting the maintenance director of the false form and only accepting the originals instead of copies.

Title: SMT ATX ACFT FLOWN WITHOUT A COMPLETED 100 HOUR INSPECTION.
Narrative: AFTER RETURNING FROM A 135 TRIP OF 3 1/2 HRS, I TURNED IN THE CLIPBOARD FOR THE ACFT. BEFORE LEAVING ON THE PREVIOUS TRIP I CHKED ALL THE PAPERWORK REALLY GOOD TO SEE IF EVERYTHING WAS CORRECT. I CHKED THE PAPERWORK EXTREMELY CLOSELY BECAUSE THE CLIPBOARD WAS A NEW ONE BECAUSE THE OLD ONE HAD BEEN LOST. AS I WAS SAYING, AFTER THE TRIP, I TURNED IN THE CLIPBOARD AND STARTED TALKING TO THE DISPATCHER AND OTHER EMPLOYEES. WHILE I WAS TALKING TO THEM I LOOKED ON THE STATUS BOARD AND FOUND THAT THE STATUS BOARD HAD NOT BEEN UPDATED TO MATCH THE CLIPBOARD. ALSO, THE NEW DISPATCHER SAID THE 100 HR HAD NOT BEEN DONE YET.
