Narrative:

Small transport X off sjc northbound and small aircraft Y southbound to hwd for localizer approach 28L. I relieved a controller at the radar handoff coordinator (ci-2) position. The last thing he did was show the 2 aircraft to the controller working ar-1 (a supervisor) and ask to make sure they were separated. The controller acknowledged it. I looked back at the scope after signing on and noted the impending error. I asked the ar-1 controller if he was taking care of it. He looked at me and said where did he come from? I advised him that he had acknowledged the last controller's pointout. He said nothing and went back to work. The aircraft were approximately 1/2-1 mi apart at the same altitude.

Title: CTLR TAKES NO ACTION TO SEPARATE OPPOSITE DIRECTION TRAFFIC AT SAME ALT RESULTING IN LESS THAN STANDARD SEPARATION.

Narrative: SMT X OFF SJC NBND AND SMA Y SBND TO HWD FOR LOC APCH 28L. I RELIEVED A CTLR AT THE RADAR HDOF COORDINATOR (CI-2) POS. THE LAST THING HE DID WAS SHOW THE 2 ACFT TO THE CTLR WORKING AR-1 (A SUPVR) AND ASK TO MAKE SURE THEY WERE SEPARATED. THE CTLR ACKNOWLEDGED IT. I LOOKED BACK AT THE SCOPE AFTER SIGNING ON AND NOTED THE IMPENDING ERROR. I ASKED THE AR-1 CTLR IF HE WAS TAKING CARE OF IT. HE LOOKED AT ME AND SAID WHERE DID HE COME FROM? I ADVISED HIM THAT HE HAD ACKNOWLEDGED THE LAST CTLR'S POINTOUT. HE SAID NOTHING AND WENT BACK TO WORK. THE ACFT WERE APPROX 1/2-1 MI APART AT THE SAME ALT.
