Approximately X years ago [company] must have fired the call sign de-confliction team. It has been a nightmare ever since. If I were to file an fsap every time I have a conflict with a call sign; I would have to file one on every leg. Today on approach control with the call sign of aircraft X; I was advised there was an aircraft Y on frequency. We had to follow them all the way in. Then aircraft Z came on frequency to add to the confusion. Someone is going to get violated or worse. Unfortunately; it's going to take a major incident to fix this. If you don't think this is a problem; just try talking on a conference call with everyone using a number and then make the numbers similar. [Company] please bring back the old call sign de-confliction team!
