Narrative:
I shipped out a used part not realizing it was hazmat. I did this because the mechanic had placed the part to be shipped on my desk in a used box and I just shipped it out not doing a hazmat rap on it. I also believe I made this mistake due to all the other duties that I was performing at the time and did not pay attention to this specific part when prepping it to be shipped.
