It is a little ironic that I had just recently read your december issue regarding 'the paper chase.' my case is verbatim of the example used! During departure TRACON asked which climb we had been given by the tower re: the jfk SID. I have to take full responsibility for deviation from the clearance! I do not go to jfk very often, just enough to remain familiar with most of the procedures and the airport layout. At least 9 out of 10 times we are given the climb that requires a climb to 2500' MSL before crossing a radial west of jfk, and then a turn to the south before crossing another radial from yet another VOR. (I do not have the charts with me at home while I am writing this, so forgive the lack of detail names for the SID.) we were given the canarsie climb, which should cross canarsie VOR. I reviewed the procedure, of course, before departure, but with some delay between that and the takeoff clearance, by the time I got the gear in the wells, I realized I was foggy about what I was supposed to do! We are creatures of habit, so what I did was follow the wrong climb procedure, thinking all along that that was the procedure we were usually given (i.e., mindset). This mindset, confusion and the very busy jfk SID got me this time! Next time I'll ask the PNF to call out at least the first 2 duties of the SID as we get the gear up!

Title: A NON COMPLIANCE WITH DEP PROC AS SMT PIC MISREADS CHART AND MINDSET TAKES OVER.
