Narrative:
ZOA issued clearance to cross tails at 11000'. I had loaded the INS with radial/DME fixes instead of fixes by name. I must have loaded tails incorrectly, because after crossing eugen the INS heading indicator showed a 90 degree right turn. We quickly reverted to VOR mode and set up tails and went directly to it. We were about 500-800' high at tails. This was because the original descent plan was based on an incorrect fix.

Title: ACR FLT CREW LOADED INCORRECT INFORMATION INTO FMS. MISSED CROSSING RESTRICTION.
Narrative: ZOA ISSUED CLRNC TO CROSS TAILS AT 11000'. I HAD LOADED THE INS WITH RADIAL/DME FIXES INSTEAD OF FIXES BY NAME. I MUST HAVE LOADED TAILS INCORRECTLY, BECAUSE AFTER XING EUGEN THE INS HDG INDICATOR SHOWED A 90 DEG RIGHT TURN. WE QUICKLY REVERTED TO VOR MODE AND SET UP TAILS AND WENT DIRECTLY TO IT. WE WERE ABOUT 500-800' HIGH AT TAILS. THIS WAS BECAUSE THE ORIGINAL DSNT PLAN WAS BASED ON AN INCORRECT FIX.
