Narrative:
When arriving san, I was advised that runway hold short lines were removed at this time (verbally by copilot after he listened to the ATIS). Later when we taxied out for takeoff we were cleared for takeoff before reaching the end of the runway. As we passed hold short lines, I noticed that they were visible and wondered if san was doing repainting work and then I noticed the heavy lines coming up, which I had not seen before at san. I assumed these lines to be centerline paint markings and pushed the throttles up and activated the automatic throttle system. I became fixated on the line while turning the aircraft and accelerating so as to place the nose gear on the line. Just at rotation as the nose was being raised, I realized I had used the side of the runway as a centerline and that the left wheel must have been just off the side of the runway.

Title: TKOF AT SAN WAS MADE STRADDLING THE LEFT SIDE RWY LIGHTS.
Narrative: WHEN ARRIVING SAN, I WAS ADVISED THAT RWY HOLD SHORT LINES WERE REMOVED AT THIS TIME (VERBALLY BY COPLT AFTER HE LISTENED TO THE ATIS). LATER WHEN WE TAXIED OUT FOR TKOF WE WERE CLRED FOR TKOF BEFORE REACHING THE END OF THE RWY. AS WE PASSED HOLD SHORT LINES, I NOTICED THAT THEY WERE VISIBLE AND WONDERED IF SAN WAS DOING REPAINTING WORK AND THEN I NOTICED THE HEAVY LINES COMING UP, WHICH I HAD NOT SEEN BEFORE AT SAN. I ASSUMED THESE LINES TO BE CENTERLINE PAINT MARKINGS AND PUSHED THE THROTTLES UP AND ACTIVATED THE AUTO THROTTLE SYS. I BECAME FIXATED ON THE LINE WHILE TURNING THE ACFT AND ACCELERATING SO AS TO PLACE THE NOSE GEAR ON THE LINE. JUST AT ROTATION AS THE NOSE WAS BEING RAISED, I REALIZED I HAD USED THE SIDE OF THE RWY AS A CENTERLINE AND THAT THE LEFT WHEEL MUST HAVE BEEN JUST OFF THE SIDE OF THE RWY.
