Narrative:

In 4/90, I discovered that my a&P #south showed up on a yellow tag of a part that was shipped to calif. Someone used my a&P #south and faked my signature. The yellow tag read something like this: 'maintenance release. I certify I have inspected this part iow current far's and find it serviceable.' I am fully aware that only certified repair stations can issue a maintenance release. A complete report has been filed with the flight standards district office in miami.

Title: MECHANIC REPORTER CLAIMS THAT HIS SIGNATURE WAS FORGED ON A PARTS TAG AND CERTIFIED AS TO THE AIRWORTHINESS OF THE ITEM ALONG WIT HIS A&P LICENSE #.

Narrative: IN 4/90, I DISCOVERED THAT MY A&P #S SHOWED UP ON A YELLOW TAG OF A PART THAT WAS SHIPPED TO CALIF. SOMEONE USED MY A&P #S AND FAKED MY SIGNATURE. THE YELLOW TAG READ SOMETHING LIKE THIS: 'MAINT RELEASE. I CERTIFY I HAVE INSPECTED THIS PART IOW CURRENT FAR'S AND FIND IT SERVICEABLE.' I AM FULLY AWARE THAT ONLY CERTIFIED REPAIR STATIONS CAN ISSUE A MAINT RELEASE. A COMPLETE RPT HAS BEEN FILED WITH THE FLT STANDARDS DISTRICT OFFICE IN MIAMI.
