Narrative:
Aircraft departed fai international and on climb out was unable to retract nose gear. Returned to ramp and it was discovered that the external nose gear lever had not been correctly positioned. This was corrected and the aircraft departed normally. Because of the extreme emphasis put on on-time operation a new flight release and weight and balance was forgotten in the rush. Subsequent departure west/O new weight and balance and new release was inadvertent due to general confusion resulting from multiple sources. Note that release factors were still valid with regards to destination, altitude, fuel requirements etc. Aircraft burned approximately 100 pounds of fuel in its circuit, giving negligible change in weight or center of gravity. Suggestions: maintenance and ground personnel have standardized training in use of ground look. Ground look be put on after start checklist. Some cockpit indication be provided of position.
Original NASA ASRS Text
Title: FLT CREW OF MDT UNABLE TO RAISE NOSE GEAR AFTER TKOF AT FAI. RETURNED AND LANDED. EXTERNAL NOSE GEAR LEVER NOT PROPERLY ALIGNED. TOOK OFF WITHOUT NEW RELEASE OR NEW WEIGHT AND BALANCE.
Narrative: ACFT DEPARTED FAI INTL AND ON CLBOUT WAS UNABLE TO RETRACT NOSE GEAR. RETURNED TO RAMP AND IT WAS DISCOVERED THAT THE EXTERNAL NOSE GEAR LEVER HAD NOT BEEN CORRECTLY POSITIONED. THIS WAS CORRECTED AND THE ACFT DEPARTED NORMALLY. BECAUSE OF THE EXTREME EMPHASIS PUT ON ON-TIME OPERATION A NEW FLT RELEASE AND WT AND BALANCE WAS FORGOTTEN IN THE RUSH. SUBSEQUENT DEP W/O NEW WT AND BALANCE AND NEW RELEASE WAS INADVERTENT DUE TO GENERAL CONFUSION RESULTING FROM MULTIPLE SOURCES. NOTE THAT RELEASE FACTORS WERE STILL VALID WITH REGARDS TO DEST, ALT, FUEL REQUIREMENTS ETC. ACFT BURNED APPROX 100 LBS OF FUEL IN ITS CIRCUIT, GIVING NEGLIGIBLE CHANGE IN WT OR CTR OF GRAVITY. SUGGESTIONS: MAINT AND GND PERSONNEL HAVE STANDARDIZED TRNING IN USE OF GND LOOK. GND LOOK BE PUT ON AFTER START CHKLIST. SOME COCKPIT INDICATION BE PROVIDED OF POSITION.
