Narrative:
On takeoff roll from mia using runway 12, extreme vibration, accompanied by several metallic thumps and an odor of burning rubber; were encountered past V1 (approximately 148 KTS). Aircraft weight was 147000 pounds. After takeoff, advised tower and asked them to check runway. They reported back pieces of tire and several pieces of metal found. Also, after takeoff, all captain heading information and first officer pdf (attitude indicator) went blank. After cleaning up stabilizing the aircraft, the heading and attitude information were recovered using an in-flight alignment procedure. After conferring with company, it was decided to fly the aircraft to atl for maintenance and passenger convenience. A climb to altitude was initiated but passing FL180, the cabin pressurization could not be controled. A descent was initiated and the decision made to stay in mia area, lower our gross weight, and land. A 500' fly-by the mia tower was conducted. They reported both nose tires shredded. This later proved wrong, only 1 was lost. After burning down to 10000 pounds fuel, an approach and normal landing was accomplished. Heavy vibrations encountered slowing through 80 KTS. Emergency had been declared and equipment was standing by. Cockpit and cabin coordination throughout were excellent, and cooperation from all involved ATC facs also excellent. After landing visual inspection showed half of the nose gear spray deflector missing, left nose tire shredded, 1 hole in the fuselage east and east compartment, 2 holes in the inboard side of the left engine cowling, and damage to 2-3 engine fan blades.

Title: ADVETCH ACR MLG SUFFERS DAMAGE ON TKOF FROM DISINTEGRATED NOSE WHEEL TIRE AND SPRAY DEFLECTOR.
Narrative: ON TKOF ROLL FROM MIA USING RWY 12, EXTREME VIBRATION, ACCOMPANIED BY SEVERAL METALLIC THUMPS AND AN ODOR OF BURNING RUBBER; WERE ENCOUNTERED PAST V1 (APPROX 148 KTS). ACFT WT WAS 147000 LBS. AFTER TKOF, ADVISED TWR AND ASKED THEM TO CHK RWY. THEY RPTED BACK PIECES OF TIRE AND SEVERAL PIECES OF METAL FOUND. ALSO, AFTER TKOF, ALL CAPT HDG INFO AND F/O PDF (ATTITUDE INDICATOR) WENT BLANK. AFTER CLEANING UP STABILIZING THE ACFT, THE HDG AND ATTITUDE INFO WERE RECOVERED USING AN INFLT ALIGNMENT PROC.
