Narrative:
The problem arose when filling out my application for my airline transport pilot license (ATP). The new FAA form added a statement under article (U) of the form requesting disclosure of a driving offense involving alcohol. Because I have filled out so many of these forms in the past I checked off, 'no', the boxes per rote/reflex action. The application had already been submitted. I did have a driving while under the influence in oct of 1981; nearly 10 yrs ago. The failure to list this seemingly meaningless event was not deliberate but a result of a failure to make airmen aware of the newly affected changes on the application form. On my latest FAA class 1 medical I did list that I had a traffic violation on my driving record, a failure to dim my headlights in august of 1990 and signed the appropriate release to allow FAA access to my records on which I have nothing to hide. This problem arose for me, I believe, because the placement of the 'new question' is connected to a felony statement regarding drugs and possession and arrest. I suggest it be moved to it's own section so it will not be overlooked by other airmen.

Title: ATP APPLICANT COMPLAINS THAT SECTION ON LICENSE APPLICATION DEALING WITH ALCOHOL OFFENSES IS NOT SUFFICIENTLY OBVIOUS.
Narrative: THE PROB AROSE WHEN FILLING OUT MY APPLICATION FOR MY AIRLINE TRANSPORT PLT LICENSE (ATP).
