Narrative:
I received a handoff on small transport X south of smf proceeding direct to ila. The aircraft subsequently lost his transponder and went into coast status and dropped off the radar scope into the coast/suspend list. This happened during position relief and the aircraft eventually exited our airspace and entered oakland center's airspace without a handoff. Contributing factors in this event were the fact that the aircraft's transponder was malfunctioning, which the previous controller did not inform me of. It happened after 2 hours and 20 mins on position, and during a position relief. It was discovered when the aircraft had his destination in sight and the controller told him to squawk VFR and good day. The pilot then informed the controller that he was IFR and did he want to make a frequency change. The center also called with an overdue aircraft report.

Title: SMT X UNCOORD AIRSPACE ENTRY ENTERED ZOA AIRSPACE WITHOUT PROPER TRACON ARTCC INTERFAC COORD. OPDEV.
Narrative: I RECEIVED A HDOF ON SMT X S OF SMF PROCEEDING DIRECT TO ILA. THE ACFT SUBSEQUENTLY LOST HIS TRANSPONDER AND WENT INTO COAST STATUS AND DROPPED OFF THE RADAR SCOPE INTO THE COAST/SUSPEND LIST. THIS HAPPENED DURING POS RELIEF AND THE ACFT EVENTUALLY EXITED OUR AIRSPACE AND ENTERED OAKLAND CENTER'S AIRSPACE WITHOUT A HDOF. CONTRIBUTING FACTORS IN THIS EVENT WERE THE FACT THAT THE ACFT'S TRANSPONDER WAS MALFUNCTIONING, WHICH THE PREVIOUS CTLR DID NOT INFORM ME OF. IT HAPPENED AFTER 2 HRS AND 20 MINS ON POS, AND DURING A POS RELIEF. IT WAS DISCOVERED WHEN THE ACFT HAD HIS DEST IN SIGHT AND THE CTLR TOLD HIM TO SQUAWK VFR AND GOOD DAY. THE PLT THEN INFORMED THE CTLR THAT HE WAS IFR AND DID HE WANT TO MAKE A FREQ CHANGE. THE CENTER ALSO CALLED WITH AN OVERDUE ACFT RPT.
