Narrative:
I wish to state I did not return this within 10 days because of difficulty in obtaining the form. I called AOPA, they referred me to gpo, boston. They told me I had to apply for it in writing. I still have not received it. A friend had a copy. He gave this to me. In terms of human performance considerations, the lesson I learned is that I have to stay on top of constantly changing circumstances and immediately implement appropriate corrective action. When I swerved off runway, I froze rather than took action to mitigate the problem. I feel this can only be achieved by constant training and experience.

Title: SMA PLT PRACTICING TOUCH AND GOES, LOSES ACFT CTL, SWERVES OFF RWY.
Narrative: I WISH TO STATE I DID NOT RETURN THIS WITHIN 10 DAYS BECAUSE OF DIFFICULTY IN OBTAINING THE FORM. I CALLED AOPA, THEY REFERRED ME TO GPO, BOSTON.
