Narrative:
Air taxi departed dannelly field runway 27 on runway heading climbing to 3000 ft. An army mlt departed airport heading 210 degree climbing to 3000 ft. I climbed the helicopter to 5000 ft on initial contact. I climbed the air taxi to 4000 ft and turned him south to separate him from another aircraft at 5000 ft. When I looked at the strips I mistakenly looked at another mlt's strip with similar call sign indicating assigned altitude of 3000 ft. I turned the air taxi on course to the east. When I looked at the scope I saw the air taxi and mlt on converging courses, at 4000 ft. I had the air taxi expedite climb to 7000 ft and the helicopter descend to 3000 ft. Standard IFR separation was lost (less that 3 mi). The air taxi reported the helicopter in sight and was instructed to maintain visual separation. Cause was lack of concentration after almost 2 hours of working heavy complex traffic. I have not worked this type of traffic in almost 3 yrs and my proficiency was not sufficient for the task.

Title: CTLR FORGETS ABOUT THE ASSIGNED ALT ISSUED TO AN ACFT AND VECTORS ANOTHER ACFT IN CLOSE PROX RESULTING IN LTSS.
Narrative: AIR TAXI DEPARTED DANNELLY FIELD RWY 27 ON RWY HDG CLBING TO 3000 FT. AN ARMY MLT DEPARTED ARPT HDG 210 DEG CLBING TO 3000 FT. I CLBED THE HELI TO 5000 FT ON INITIAL CONTACT. I CLBED THE AIR TAXI TO 4000 FT AND TURNED HIM S TO SEPARATE HIM FROM ANOTHER ACFT AT 5000 FT.
