Narrative:
San juan approach cleared us for a 'visual runway 8.' after landing they said they had cleared us for a lagoon visual to 8, wanted to know why we made the wrong visual approach. We both heard the controller say 'visual runway 8' and the copilot read back 'visual 8.' after we cleared the runway and requested taxi to 'airport services,' they said 'taxi to the ramp.' we saw we were going to wind up nose to nose with a large transport so I pulled aside to let the large transport pass, wasn't a problem. When I queried ground they said they never cleared us anywhere, that we didn't call. On departing sju with a patient on board/lifeguard flight, we got our clearance 20 min in advance. While taxiing out they said we didn't have any IFR clearance. We wound up refiling, took about 5 mins. Callback conversation with reporter revealed the following information: reporter states this all seemed very strange. Normally no problem with sju. Everything went wrong this day. Flight crew positive about approach clearance or would have confirmed. Did a readback of taxi clearance. How could they readback if not issued one? Copied departure clearance, how could they if not on file and issued to them? No language problem, so don't know what was happening with ground control.

Title: AIR AMB HAS COM DIFFICULTY AT SJU ARPT. WRONG VISUAL APCH, SUPPOSEDLY NO TAXI CLRNC, DEPARTING FLT PLAN 'LOST' AFTER CLRNC ISSUED.
Narrative: SAN JUAN APCH CLRED US FOR A 'VISUAL RWY 8.' AFTER LNDG THEY SAID THEY HAD CLRED US FOR A LAGOON VISUAL TO 8, WANTED TO KNOW WHY WE MADE THE WRONG VISUAL APCH. WE BOTH HEARD THE CTLR SAY 'VISUAL RWY 8' AND THE COPLT READ BACK 'VISUAL 8.' AFTER WE CLRED THE RWY AND REQUESTED TAXI TO 'ARPT SVCS,' THEY SAID 'TAXI TO THE RAMP.' WE SAW WE WERE GOING TO WIND UP NOSE TO NOSE WITH A LGT SO I PULLED ASIDE TO LET THE LGT PASS, WASN'T A PROBLEM. WHEN I QUERIED GND THEY SAID THEY NEVER CLRED US ANYWHERE, THAT WE DIDN'T CALL. ON DEPARTING SJU WITH A PATIENT ON BOARD/LIFEGUARD FLT, WE GOT OUR CLRNC 20 MIN IN ADVANCE. WHILE TAXIING OUT THEY SAID WE DIDN'T HAVE ANY IFR CLRNC. WE WOUND UP REFILING, TOOK ABOUT 5 MINS. FLC POSITIVE ABOUT APCH CLRNC OR WOULD HAVE CONFIRMED. DID A READBACK OF TAXI CLRNC. HOW COULD THEY READBACK IF NOT ISSUED ONE? COPIED DEP CLRNC, HOW COULD THEY IF NOT ON FILE AND ISSUED TO THEM? NO LANGUAGE PROBLEM, SO DON'T KNOW WHAT WAS HAPPENING WITH GND CTL.
