Narrative:
Flight crew rejected the takeoff immediately after applying power due to a takeoff warning horn sounding. Horn sounded due to 1 degree takeoff flap being selected instead of 5 degree standard flap setting for takeoff on the medium large transport aircraft is 1 degree min versus 5 degree min for the medium large transport 4 aircraft. Even an experienced crew can make this mistake while consciously completing the checklist. There are no placards in the cockpit dictating 5 degree flap min for the medium large transport 4 aircraft and the checklist is the same for both versions of the aircraft. It is easy to forget what aircraft you are in since the medium large transport 3 and medium large transport 4 are intermixed in the daily flying schedule and aircrew routinely fly both models in the same day.
Original NASA ASRS Text
Title: FLC INADVERTENTLY STARTED TKOF WITH THE WRONG WING FLAP SETTING RESULTING IN AN ABORTED TKOF.
Narrative: FLC REJECTED THE TKOF IMMEDIATELY AFTER APPLYING PWR DUE TO A TKOF WARNING HORN SOUNDING. HORN SOUNDED DUE TO 1 DEG TKOF FLAP BEING SELECTED INSTEAD OF 5 DEG STANDARD FLAP SETTING FOR TKOF ON THE MLG ACFT IS 1 DEG MIN VERSUS 5 DEG MIN FOR THE MLG 4 ACFT. EVEN AN EXPERIENCED CREW CAN MAKE THIS MISTAKE WHILE CONSCIOUSLY COMPLETING THE CHKLIST. THERE ARE NO PLACARDS IN THE COCKPIT DICTATING 5 DEG FLAP MIN FOR THE MLG 4 ACFT AND THE CHKLIST IS THE SAME FOR BOTH VERSIONS OF THE ACFT. IT IS EASY TO FORGET WHAT ACFT YOU ARE IN SINCE THE MLG 3 AND MLG 4 ARE INTERMIXED IN THE DAILY FLYING SCHEDULE AND AIRCREW ROUTINELY FLY BOTH MODELS IN THE SAME DAY.
