Narrative:
I received a call from operations advising of a pending flight. I asked the basic where and when questions and determined that the flight could be accomplished within my available duty time. At XX20 after the medical passengers had taken report and given report to their medical command we departed hospital a. We landed at airport at XX34 expecting a 20-30 min turnaround. 1 hour and 21 mins later we departed for hospital B. The passengers stated that their patient was much more serious than originally reported to include a full body cast that was not expected. Even with this they did not expect the delay to be long, but complications developed. The patient was extremely critical his main complaint being a cerebral bleed. Because of the patient's condition the option of waiting for the relief pilot was not considered. We landed at hospital B at XY18 and 27 mins later departed for hospital a. We landed at hospital a at XZ23 having overflown our duty time by 23 mins.
