Narrative:

At PM45 hours on 6/wed/92, an a and P mechanic, checked the oil on the right engine of our light transport aircraft. When he closed the cowling, he was having difficulty properly latching the cowling. He then had to use a ladder so that he could press down firmly on the cowling to secure it properly. We then flew the aircraft for the remainder of the day without incident. That night the aircraft was left in monroe, la, for an LC2 and a turbine wash. The following morning, during the preflight, the cowling appeared to be properly secured. At AM33 hours on 6/wed/92, during takeoff from monroe, la, the cowling came open at 500 ft AGL. We then immediately returned to monroe, la, to resecure the cowling. Upon immediate inspection by the a and P mechanics in monroe, the cowling was considered undamaged and it was then resecured for additional flight. We then flew the aircraft to memphis, tn. Once in memphis, the a and P mechanics in mem inspected the cowling and they considered it too damaged to utilize for any additional flight. Supplemental information from acn 214111. Title: A RETURN LAND SITUATION EVOLVES AFTER ACR LTT R HAND COWLING OPENS AFTER TKOF.

Narrative: AT PM45 HRS ON 6/WED/92, AN A AND P MECH, CHKED THE OIL ON THE R ENG OF OUR LTT ACFT. WHEN HE CLOSED THE COWLING, HE WAS HAVING DIFFICULTY PROPERLY LATCHING THE COWLING. HE THEN HAD TO USE A LADDER SO THAT HE COULD PRESS DOWN FIRMLY ON THE COWLING TO SECURE IT PROPERLY. WE THEN FLEW THE ACFT FOR THE REMAINDER OF THE DAY WITHOUT INCIDENT. THAT NIGHT THE ACFT WAS LEFT IN MONROE, LA, FOR AN LC2 AND A TURBINE WASH. THE FOLLOWING MORNING, DURING THE PREFLT, THE COWLING APPEARED TO BE PROPERLY SECURED. AT AM33 HRS ON 6/WED/92, DURING TKOF FROM MONROE, LA, THE COWLING CAME OPEN AT 500 FT AGL. WE THEN IMMEDIATELY RETURNED TO MONROE, LA, TO RESECURE THE COWLING. UPON IMMEDIATE INSPECTION BY THE A AND P MECHS IN MONROE, THE COWLING WAS CONSIDERED UNDAMAGED AND IT WAS THEN RESECURED FOR ADDITIONAL FLT. WE THEN FLEW THE ACFT TO MEMPHIS, TN. ONCE IN MEMPHIS, THE A AND P MECHS IN MEM INSPECTED THE COWLING AND THEY CONSIDERED IT TOO DAMAGED TO UTILIZE FOR ANY ADDITIONAL FLT. SUPPLEMENTAL INFO FROM ACN 214111.
