Narrative:
Engine and propeller de-ice failed which resulted in an engine flameout when de-ice was reset with its circuit breaker. Engine (lh) was relighted normally after flameout due to ice ingestion with ignition on. Unfortunately, I failed to notify ATC of the engine flameout due to the pace required to restart the left engine. The left engine was the only engine and propeller affected by the (lh) de-ice failure.

Title: ACR MDT ACFT ENG FLAMED OUT DUE TO ENG DEICE EQUIP MALFUNCTION. INFLT ENG SHUTDOWN.
Narrative: ENG AND PROP DE-ICE FAILED WHICH RESULTED IN AN ENG FLAMEOUT WHEN DE-ICE WAS RESET WITH ITS CIRCUIT BREAKER. ENG (LH) WAS RELIGHTED NORMALLY AFTER FLAMEOUT DUE TO ICE INGESTION WITH IGNITION ON. UNFORTUNATELY, I FAILED TO NOTIFY ATC OF THE ENG FLAMEOUT DUE TO THE PACE REQUIRED TO RESTART THE L ENG. THE L ENG WAS THE ONLY ENG AND PROP AFFECTED BY THE (LH) DE-ICE FAILURE.
