Narrative:

Flight mia-uio. On descent into uio the pressurization was not reset in the descent. Departure flight information of 10 ft was set and the landing airport information was set plus or minus 9300 ft. On running the parking checklist the crew noted that the cabin was still at sea level pressure. At the same time the ground crew was unable to open the forward cabin door due to pressure lock. The crew then initiated manual control of the pressurization system and set approximately 2000 FPM rate of climb for the cabin. When the cabin pressure reached 4500 ft the cabin depressurized completely to 9300 ft. The probable reason that the pressurization was not reset on descent was that the crew were revising the approach procedures which are very intricate for quito. The procedure turn outbound starts at 17000 ft. This could have been a problem if an emergency evacuate/evacuation was required and the doors could not be opened immediately.

Title: FLC OF ACR LGT ACFT FAILED TO SET THE LNDG ARPT ALT IN THE ACFT PRESSURIZATION SYS RESULTING IN THE ACFT NOT BEING COMPLETELY DEPRESSURIZED WHEN AN ATTEMPT WAS MADE TO OPEN THE CABIN DOOR AFTER LNDG.

Narrative: FLT MIA-UIO. ON DSCNT INTO UIO THE PRESSURIZATION WAS NOT RESET IN THE DSCNT. DEP FLT INFO OF 10 FT WAS SET AND THE LNDG ARPT INFO WAS SET PLUS OR MINUS 9300 FT. ON RUNNING THE PARKING CHKLIST THE CREW NOTED THAT THE CABIN WAS STILL AT SEA LEVEL PRESSURE. AT THE SAME TIME THE GND CREW WAS UNABLE TO OPEN THE FORWARD CABIN DOOR DUE TO PRESSURE LOCK. THE CREW THEN INITIATED MANUAL CTL OF THE PRESSURIZATION SYS AND SET APPROX 2000 FPM RATE OF CLB FOR THE CABIN. WHEN THE CABIN PRESSURE REACHED 4500 FT THE CABIN DEPRESSURIZED COMPLETELY TO 9300 FT. THE PROBABLE REASON THAT THE PRESSURIZATION WAS NOT RESET ON DSCNT WAS THAT THE CREW WERE REVISING THE APCH PROCS WHICH ARE VERY INTRICATE FOR QUITO. THE PROC TURN OUTBOUND STARTS AT 17000 FT. THIS COULD HAVE BEEN A PROB IF AN EMER EVAC WAS REQUIRED AND THE DOORS COULD NOT BE OPENED IMMEDIATELY.
