Narrative:
After a sky diving fatality at 57C airport in the city of east troy, state of wi, I received a phone call from the milwaukee FSDO office in regard to this fatality investigation. Upon arriving at the milwaukee FSDO days later, it was brought to my attention by the acting investigator who inspected the aircraft earlier that it had a seat belt missing from an assigned floor station. As I understood, the paper work on this small aircraft had a supplemental type certificate for 1 pilot and pilot seat and 4 floor stations for 4 sky divers for a total of 5 occupants. I completed a preflight inspection prior to placing the aircraft on the flight line. (I cycled 1 load before the fatality flight and 2 after.) I didn't notice not was I told of anything unusual or missing. As far as I am concerned all seat belts were secured and at their proper stations. It was also brought to my attention that the weight and balance papers might not have been located inside the aircraft. I also want to make clear that this fatality was in no way, shape or form related to the condition of the aircraft or pilot.

Title: SKY DIVE ACFT DISCOVERED TO HAVE MISSING SEAT BELT FOR JUMPER. WT AND BAL PAPERS NOT IN ACFT.
Narrative: AFTER A SKY DIVING FATALITY AT 57C ARPT IN THE CITY OF E TROY, STATE OF WI, I RECEIVED A PHONE CALL FROM THE MILWAUKEE FSDO OFFICE IN REGARD TO THIS FATALITY INVESTIGATION. UPON ARRIVING AT THE MILWAUKEE FSDO DAYS LATER, IT WAS BROUGHT TO MY ATTN BY THE ACTING INVESTIGATOR WHO INSPECTED THE ACFT EARLIER THAT IT HAD A SEAT BELT MISSING FROM AN ASSIGNED FLOOR STATION. AS I UNDERSTOOD, THE PAPER WORK ON THIS SMA HAD A SUPPLEMENTAL TYPE CERTIFICATE FOR 1 PLT AND PLT SEAT AND 4 FLOOR STATIONS FOR 4 SKY DIVERS FOR A TOTAL OF 5 OCCUPANTS.
