Narrative:
The aircraft, an medium large transport, was brought into reno, nv, for normal maintenance on may/sun/93 at PM05 local. The ADF system had been deferred. On may/mon/93 the flight crew arrived at the airport at AM15. After checking the logbook it was noted that the ADF had been repaired. Performed normal exterior preflight, no abnormalities were noted. Aircraft left the gate at AM00 local and departed for sfo at AM06. En route, the ADF system was observed as being inoperative. This was noted in the logbook and company operations was advised. Upon arrival in sfo, maintenance personnel informed me that the left rear wheel well fairing was missing. Company operations personnel were notified. The missing wheel well fairing was found 2 mi off the end of the runway at reno. During a normal preflight, it is impossible to check to see if all the fasteners are fastened (one would have to crawl on one's hands and knees to see all the fasteners). I believe that the fasteners on the bottom of the fairing were left undone by maintenance personnel in reno.

Title: RPTR COMPLAINS OF FACILITY ACFT MAINT DISCREPANCY PROCS THAT PRODUCED ACFT EQUIP PROBS.
Narrative: THE ACFT, AN MLG, WAS BROUGHT INTO RENO, NV, FOR NORMAL MAINT ON MAY/SUN/93 AT PM05 LCL. THE ADF SYS HAD BEEN DEFERRED. ON MAY/MON/93 THE FLC ARRIVED AT THE ARPT AT AM15. AFTER CHKING THE LOGBOOK IT WAS NOTED THAT THE ADF HAD BEEN REPAIRED. PERFORMED NORMAL EXTERIOR PREFLT, NO ABNORMALITIES WERE NOTED. ACFT LEFT THE GATE AT AM00 LCL AND DEPARTED FOR SFO AT AM06. ENRTE, THE ADF SYS WAS OBSERVED AS BEING INOP. THIS WAS NOTED IN THE LOGBOOK AND COMPANY OPS WAS ADVISED. UPON ARR IN SFO, MAINT PERSONNEL INFORMED ME THAT THE L REAR WHEEL WELL FAIRING WAS MISSING. COMPANY OPS PERSONNEL WERE NOTIFIED. THE MISSING WHEEL WELL FAIRING WAS FOUND 2 MI OFF THE END OF THE RWY AT RENO. DURING A NORMAL PREFLT, IT IS IMPOSSIBLE TO CHK TO SEE IF ALL THE FASTENERS ARE FASTENED (ONE WOULD HAVE TO CRAWL ON ONE'S HANDS AND KNEES TO SEE ALL THE FASTENERS). I BELIEVE THAT THE FASTENERS ON THE BOTTOM OF THE FAIRING WERE LEFT UNDONE BY MAINT PERSONNEL IN RENO.
