Narrative:

Flight went over to approach and was told runway 25L RVR was up to 3500 ft and we were cleared for the approach to runway 25L. Since we were 25 mi out, we asked lax to confirm approach clearance and he did. At this time we changed from a CAT ii to a CAT I approach. This means that instead of the first officer flying, the captain flew the approach. Due to changing WX and changing roles of who was flying, we failed to change from our final monitor on 124.9 to tower on 120.95 for landing clearance. After clearing runway 25L we called tower for clearance to cross runway 25R. Due to not calling tower until clear of runway 25L the tower lost a departure slot on runway 25R.

Title: FLC OF AN MLG ACR ACFT INADVERTENTLY LANDED WITHOUT TWR CLRNC. OUT OF AN ILS APCH TO MINIMUMS.

Narrative: FLT WENT OVER TO APCH AND WAS TOLD RWY 25L RVR WAS UP TO 3500 FT AND WE WERE CLRED FOR THE APCH TO RWY 25L. SINCE WE WERE 25 MI OUT, WE ASKED LAX TO CONFIRM APCH CLRNC AND HE DID. AT THIS TIME WE CHANGED FROM A CAT II TO A CAT I APCH. THIS MEANS THAT INSTEAD OF THE FO FLYING, THE CAPT FLEW THE APCH. DUE TO CHANGING WX AND CHANGING ROLES OF WHO WAS FLYING, WE FAILED TO CHANGE FROM OUR FINAL MONITOR ON 124.9 TO TWR ON 120.95 FOR LNDG CLRNC. AFTER CLRING RWY 25L WE CALLED TWR FOR CLRNC TO CROSS RWY 25R. DUE TO NOT CALLING TWR UNTIL CLR OF RWY 25L THE TWR LOST A DEP SLOT ON RWY 25R.
