Narrative:

I worked the evening shift at salt lake TRACON on jan/thu/94. Almost every time I worked a radar position, I noticed multiple targets that had lost radar data, beacon data, or both! The worst happened around XX30 pm while on final position. This is when the data is needed the most because the aircraft are in close proximity. This has been an ongoing problem since the ASR-9 was installed. Many controllers here don't fill these forms because they don't think anything will change and are afraid of the chance of their name being turned in.
