Narrative:

During climb aircraft would not pressurize both automatic and man modes were tried with no effect. During post-flight inspection at dtw, first officer noticed access panel for hell hole was missing. Contacted maintenance in cvg. When informed of the missing panel, they checked in the hangar, and found the cover. They said the mechanic forgot to install the panel after completing maintenance. One compelling question, why didn't the first officer notice. We accepted the aircraft in the maintenance hangar. The airplane was backed into parking and the first officer would have had to climb through deep snow drifts to see the area of the missing panel. Supplemental information from acn 261738: contributing factors: first officer was not aware maintenance had worked on that part of aircraft. Aircraft parked on ramp with tail over hanging a large snow bank. So missing panel was not visible on preflight.

Title: UNABLE TO PRESSURIZE. REAR PRESSURE DOOR MISSING.

Narrative: DURING CLB ACFT WOULD NOT PRESSURIZE BOTH AUTO AND MAN MODES WERE TRIED WITH NO EFFECT. DURING POST-FLT INSPECTION AT DTW, FO NOTICED ACCESS PANEL FOR HELL HOLE WAS MISSING.
