Narrative:
After takeoff at den runway 35L approximately 8000 ft MSL, #3 engine exploded and shut itself down. This was immediately after flap retraction. Emergency was declared with den approach. Accomplished all appropriate emergency and abnormal procedures. Dumped approximately 20000 pounds of fuel to get down to landing weight. Returned to airport and made normal landing on runway 35R at maximum landing weight. After landing, emergency personnel inspected rear of aircraft for damage and finding none, taxied back to gate and deplaned passenger. Prior to engine failure, all engine indications were normal. Engine failure was so violent, the #3 throttles slammed close on its own. All the cowl latches were knocked open, and the tailpipe had numerous dents from metal exiting the engine. As of this date have not heard what caused failure.
Original NASA ASRS Text
Title: ENG EXPLOSION AND AUTOMATIC SHUTDOWN OF #3 ENG LGT DURING INITIAL CLB. EMER WAS DECLARED AND A RETURN LAND WAS SUCCESSFULLY MADE AFTER FUEL WAS DUMPED.
Narrative: AFTER TKOF AT DEN RWY 35L APPROX 8000 FT MSL, #3 ENG EXPLODED AND SHUT ITSELF DOWN. THIS WAS IMMEDIATELY AFTER FLAP RETRACTION. EMER WAS DECLARED WITH DEN APCH. ACCOMPLISHED ALL APPROPRIATE EMER AND ABNORMAL PROCS. DUMPED APPROX 20000 LBS OF FUEL TO GET DOWN TO LNDG WT. RETURNED TO ARPT AND MADE NORMAL LNDG ON RWY 35R AT MAX LNDG WT. AFTER LNDG, EMER PERSONNEL INSPECTED REAR OF ACFT FOR DAMAGE AND FINDING NONE, TAXIED BACK TO GATE AND DEPLANED PAX. PRIOR TO ENG FAILURE, ALL ENG INDICATIONS WERE NORMAL. ENG FAILURE WAS SO VIOLENT, THE #3 THROTTLES SLAMMED CLOSE ON ITS OWN. ALL THE COWL LATCHES WERE KNOCKED OPEN, AND THE TAILPIPE HAD NUMEROUS DENTS FROM METAL EXITING THE ENG. AS OF THIS DATE HAVE NOT HEARD WHAT CAUSED FAILURE.
