Narrative:
I have been employed with air carrier X as a dispatcher since feb/85. On aug/tue/94 I had a flight depart where a component was put on MEL. This component was a legal MEL item, however, an amendment to the release was necessary for further legal operation per air carrier X flight operations manual, flight service manual, and FARS. Our maintenance control nor the flight crew contacted or advised me, the dispatcher of record, of the MEL. Enclosed, you will find an e-mail message that is fairly self explanatory. As you are aware, maintenance issued MEL number X reference 28-05 for flight Y aircraft Z on the ground in pdx on aug/tue/94. They overlooked advising me or the supervisor on duty of this action. The crew also overlooked calling for an amendment required on the reserve fuel which would increase from 2700 pounds to 2900 pounds. Flight Y then flew pdx- smf, leg 3 of the release of which leg 2 had been previously flown, where the APU went inoperative and was put on MEL. At the time, I was advised of the APU MEL, I was in the process of working up another release due, and noticed the MEL on the fuel quantity indicator and approached maintenance controller and my supervisor. At this time, I contacted the crew via SELCAL and amended leg 4 of the release to read re 2900 and mt 9300. Also as you are aware, I contacted 'a' the scheduled duty officer, and advised him of what occurred. A said he would talk to you, I also gave 'B' a copy of the release.
