Narrative:

A review of aircraft records in the aim showed the aircraft was 4.9 hours away from the 25 hour inspection. I observed the mechanic doing the inspection and assumed he signed off the 25 hour. However, he was interrupted by phone calls and multiple tasks that made it impossible for him to get his job done. Our maintenance department was aware of several aircraft movements and did not return to complete an inspection during the day. I was not aware that the 25 hour was not complete until I was on my 11TH flight leg, 10 hours after I began flying. I was en route with patient on board when I was told that the mechanic was asking how much I had flown and how much time was left to the 25 hour. Cause: my assumption that the mechanic signed off inspection. Contributing: maintenance did not track and follow up.

Title: EMS OP FLEW HELI BEYOND 25 HR INSPECTION PERIOD.

Narrative: A REVIEW OF ACFT RECORDS IN THE AIM SHOWED THE ACFT WAS 4.9 HRS AWAY FROM THE 25 HR INSPECTION. I OBSERVED THE MECH DOING THE INSPECTION AND ASSUMED HE SIGNED OFF THE 25 HR. HOWEVER, HE WAS INTERRUPTED BY PHONE CALLS AND MULTIPLE TASKS THAT MADE IT IMPOSSIBLE FOR HIM TO GET HIS JOB DONE. OUR MAINT DEPT WAS AWARE OF SEVERAL ACFT MOVEMENTS AND DID NOT RETURN TO COMPLETE AN INSPECTION DURING THE DAY. I WAS NOT AWARE THAT THE 25 HR WAS NOT COMPLETE UNTIL I WAS ON MY 11TH FLT LEG, 10 HRS AFTER I BEGAN FLYING. I WAS ENRTE WITH PATIENT ON BOARD WHEN I WAS TOLD THAT THE MECH WAS ASKING HOW MUCH I HAD FLOWN AND HOW MUCH TIME WAS LEFT TO THE 25 HR. CAUSE: MY ASSUMPTION THAT THE MECH SIGNED OFF INSPECTION. CONTRIBUTING: MAINT DID NOT TRACK AND FOLLOW UP.
