Narrative:

While cruising at FL270, center advised us that we were 400 ft low. Upon looking at the captain's altimeter I noticed that it was set to the current barometric pressure setting instead of 29.92. I made the correction without incident. We were rushed due to an ill passenger and thus missed the FL180 checklist. In the future we must not get distracted from the task at hand.
